Drove there. Our room is located first floor with private onsen bath in the room. No shoes are allowed when you first step into the Ryokan. Slippers are provided.
Staff are friendly and nice, the room is clean and spacious. Food was typical Japanese breakfast. The private in-house onsen bath was awesome but sadly on the second day, the onsen bath tub started to leak hot spring water from all sides of the bath tub and from the wall and there is no way to stop the water gushing out!
As a kind gesture, the manager gave us some private time on the private onsen (require booking)

Transportation is a bit inconvenient, and Google Maps has some problems. To summarize: Take JR from Hakata Station to Takeo Onsen, then take the local bus to Ureshino Onsen Bus Stop (Google Navigation takes me to the Hot Springs Retreat Center Station, but I actually got off three stops earlier, so I walked for nearly an hour), and then walk for 5 minutes.
Facilities: The room is very clean, and you can make a private bath for free, with someone waiting to lead you.
